Admissions Application

Thank You for your interest in applying for admissions to Lafayette Lutheran School. There are two options for completing an application.

Choose the fill-in application to complete on the computer, save as a word document, and submit via email, or as a printed document to mail or fax in. Choose the pdf application to print and fill out by hand.

Applications are accepted year-round.

 

Documents Needed

  • Copy of Birth Certificate
  • Copy of Immunization Record
  • Copy of Current Report Card
  • Copy of Current Test Scores
  • Copy of Professional Diagnostic Testing if applicable
